Level 1 Food Truck Vendor Application

This donation constitutes the Merchant’s authorization for use of Automated Clearing House (“ACH”) debits and credits for payments between the Company and the Merchant.

The Merchant hereby authorizes The Giglio Society of East Harlem, via the ACH System to disburse credit-card receivables and make any other payments which may become due to the Merchant from the Company to collect any and all fees and chargebacks which become due and payable to The Giglio Society of East Harlem and solely for these purposes, including without limitation the correcting of any erroneous entries to initiate ACH Credit Entries and Debit Entries to the Merchant’s Credit Card Account

Full payment donation on my credit card to secure my food truck vending spot during the Giglio Society of East Harlem Festival Thursday August 8th – Sunday August 11th 2024 in the amount of: $700.00 ($USD)

I authorize The Giglio Society of East Harlem to charge the agreed amount listed above to my credit card provided herein. I agree that I will pay for this purchase in accordance with the issuing bank cardholder agreement.
